Guided Reading Lesson Plan & Small Group Templates

Plan and run small-group reading on one-page blackline templates: a lesson plan for one group with before, during and after reading, word work and a student notes list; a week at a glance for 3–6 groups; anecdotal notes boxes; a running record form with accuracy and self-correction formulas; and a rotation board for your stations. Type your student and group names to print them on the page.

Every template fills one letter page in black and white. The lesson plan prints six reading prompts to tick off; the rotation board gives each group a different starting station, one rotation per station.
