Kindergarten & Pre-K Assessment Sheets Generator (Letters, Sounds, Numbers, Shapes & Sight Words)

Print a one-on-one assessment for any early skill in two pages: a student page with the items set large in a mixed-up order to point at and name, and a teacher recording sheet with the same items in the same order, a check box for each of 3 or 4 testing dates, a score out of the total and room for notes. Choose uppercase or lowercase letter names, letter sounds, numbers to 10, 20 or 30, counting by tens, dot sets to count, 2D and 3D shapes, color words, Dolch sight words, CVC words or nonsense words. A name and number writing sample, a student summary report and a class data grid complete the assessment binder. Every page is black and white.

Put the student page in front of the child and keep the recording sheet on your clipboard: both list the items in the same order (the student page reads across, the recording sheet reads down). Check a box for each correct answer, then write the date and the score at the top. Forms A, B and C are three fixed mixed-up orders: use one per testing date so nobody memorizes the page, and reprint either page any time knowing it still matches the other.
